In this section, you will attempt to change the element that you identified in the prioritizing exercise in QUARTER TWO.

You will apply the classical Model for Improvement pictured on the left.

The Model for Improvement asks you to answer three essential questions.

  1. What are you trying to accomplish (Your SMART Aim)?

  2. How will you know that a change is an improvement (which describes the measurements you will need)?

  3. What changes can we make that will result in improvement (Answered by a series of rapid cycle tests of change)?

Instead of reading, you will first click on the How to Improve Institute for Healthcare Improvement (IHI)” link and go through the online lessons.

  1. You need to complete four sections of the online lessons:

    1. Introduction

    2. Forming the Team

    3. Setting Aims

    4. Establishing Measures

Then, click on the Q3 packet and create your first rapid cycle test of change.
